Fee, form, filing office and yearly filing for each state come from the official sources cited on that state’s page, read in October 2026. Nine fees (Arkansas, California, Georgia, Hawaii, Indiana, Louisiana, Missouri, Nebraska and New York) were re-read on October 28, 2026 because two sources disagreed; the note on those pages says which figure applies to which filing route.Service prices
